Nationwide Children's - Toledo Walk-in Sports Physicals

Nationwide Children’s - Toledo Sports Medicine is offering convenient, walk-in sports physicals for student-athletes on July 13, July 20 and July 27 from 5 to 7 p.m. at Sylvania Outpatient Specialty Clinics (7640 W. Sylvania Ave., Suite E Sylvania, OH 43560). No appointment is needed!
The cost is $25 self-pay due at the time of service and must be paid by debit or credit card. Patients should bring their 2026-2027 OHSAA PPE form and photo ID to their visit and patients under the age of 18 must be accompanied by a parent or guardian.
