Columbian Award Application (#SP-7) Due

The following forms and applications are due soon for K of C members:

1. **Columbian Award Application (#SP-7)**
2. **Service Program Personnel Report (#365)** - This can be submitted via the Service Personnel Screen on Member Management.
3. **Report of Chosen Officers for the Term (#185)** - This can also be submitted through the Officers Screen on Member Management.
4. **Food for Families Refund and Plaque Application (#10057)**
5. **RSVP Refund and Plaque Application (#2863)**

Please ensure that all completed forms are sent to the Supreme Council, your District Deputy, and the Ohio State Council. K of C Forms are available online at the Supreme Council website or through the Council Report Forms booklet. Note: The Assembly Forms, specifically Report of Officers Form (#186), is due on July 1st.
